Additional insurance is required if:

  1. Non-Girl Scouts are attending an event
  2. A trip lasting longer than 3 days and 2 nights 
    NOTE : for trips occurring during a federal holiday - 4 days and 3 nights are allowed without additional insurance.

For approval, this form must be received by council no later than 14 business days prior to your event.
Non-compliance with this deadline may mean your event cannot be insured and will have to be canceled.

Please be aware that your payment is due with the submission of this request via the online credit card payment portion of this form.
